The following documents are required for services at Beyond the Curve Pediatrics
- Client Registration Packet and Behavior Assessment
- Proof of diagnosis
- Referral from primary physician
- Copy of caregiver's ID
- copy of client's insurance(s)
- Any active Individualized Education Plans (IEPs), speech, and occupational plans
