Frequently Asked Questions

Once you are finished completing the form, save your changes to your desktop by clicking save button in top left corner. Open email and attach form on desktop to email. Send to beyondthecurvepedatrics@gmail.com. 

Pictures of small items such as insurance cards and IDs are acceptable. ALL other documents must be sent via PDF when emailing or submitted in person. 

Your child's primary care physician must refer clients for ABA therapy - to evaluate and treat. You can call the physician's office and request this or schedule an appointment with them. Referrals from Early Steps are accepted as well. 

You can refer a patient by sending a referral via fax to 337-468-3440 or by completing the form on our referral page in the navigation bar. 

Our days and hours of operation are: Mondays-Fridays 8:00-4:00. Therapy takes place during this time. Drop off is between 7:50 am and 8:00 am. Pickup is between 3:50-4:00pm. Parents do not stay onsite with clients. Each client's schedule is decided based on client need and clinic availability. We also work with school and other therapy schedules.

Yes, there is a waiting list at all locations: Ville Platte, Mamou, and Crowley. It can take 1-6 months for availability to arise at each location.
